Clinical Documentation

ALL clinical documentation for Fall Semester 2015 (this includes any documentation expiring August 24, 2015 - December 4, 2015) must be updated and turned into the office manager by noon on June 30, 2015.  Paperwork can be mailed to SUU Department of Nursing, Attention: Teresa Higbee, 351 West University Blvd., Cedar City, UT 84720 or e-mailed to higbeet@suu.edu

PN and RN Licensing

Application Process 

1. RN Candidates, Request an SUU Transcript from "My Portal", select "After Degree is Posted" and send to doplbureau7@utah.gov.
    PN Candidates, pick up a Letter of Equivalency from the Department of Nursing after passing the PN-Predictor (to be enclosed with your
    application to DOPL).
2. Register and pay Pearson Vue.
3. Once transcripts are sent (or PN-Letter of Equivalency is obtained), send to Occupational and Professional Licensing (DOPL) the completed application, required fee and completed fingerprint cards.

(Please note, test must be taken within 30 days of DOPL receiving application!

Please note: PN assessment exam for SUU nursing students (must complete level 2 successfully) will only be offered twice a year; once in May and  December. Next exam will be on December 14, 2015, 9:00 a.m. - Noon, SC301. Fee for exam $50 for assessment only or $80 for assessment and practice assessment. Students must pay and pre-register through the nursing office no later than Noon on November 13, 2015 (fees are non-refundable and are subject to change). Contact the Nursing Office for additional information. Students can pay on-line. LPN Predictor (Bring receipt to office after paying).

Level 1 Fall 2015

ALL CLINICAL DOCUMENTATION IS DUE TO TERESA BY NOON ON TUESDAY, June 30, 2015

Register for the following NURS courses: 3120, 3130, 3140, 3160 and one of the 3135 sections. You must be flexible – there will be class or clinical experiences everyday; Monday through Friday, in this first semester.
