Complete the Graduation Paperwork

Fall Completers:

These deadlines give you a semester to complete any overlooked courses the worksheet shows you're lacking. While these deadlines are the last dates to turn in worksheets, they are certainly not the first. We encourage you to turn the paperwork in as early as possible, and you'll be approved a lot sooner if you avoid the deadline rush.

Graduation Worksheets:

The worksheet serves as a guide for the registrar in determining your eligibility for graduation. It shows what completion requirements you've filled and what ones you're still lacking. Once you've completed the worksheet, have it signed by your advisor and department chair and dean.

Graduation worksheets are available and can be acquired as follows:

Bachelor Degree
Catalog graduation requirements are only good for 6 years.  After that, the student will need to take his/her graduation requirements from a more recent catalog. You can pick up a blank worksheet at the Registrar's Office or online below:

Associate Degree (AA/AS)

Associate of Applied Science (AAS)
Available at the respective departments.

Masters Degree
Available at the respective graduate program offices.

College Specific Forms

These forms are in Adobe PDF format. You will need to type in the information, print out the form, get the appropriate signatures and hand them in to the Registrar's office.

Certificate of Completion Forms:

Bachelor Degree students:
This form is available at the Registrar’s Office or download the Certificate of Completion

Masters Degree students:
The Certificate of Completion form is provided by the specific graduate program when the completed Graduation Worksheet is approved by the respective graduate program and college.

Application for Graduation Forms:

Bachelor / Associates Degrees:
This form is available at the Registrar’s Office

Masters Degree:
Masters Degree Program students can pick up their Application for Graduation (i.e., “diploma card”) from their respective graduate program office.
