Enrollment Verifications
Please fill out the Enrollment Verification Form below. If you are unable to access it, you may call, or visit the Registrar's Office and request one with your Photo ID. Or you may fax or mail a letter of request with the information noted below. Please allow 2 business days for processing. No fee required. If you would like your GPA included, you must submit your request in writing with your signature included.

Letter of Request Information:
If you are unable to access the Enrollment Verification Form, or prefer to write a letter, please include the following information:
- Signature, if GPA requested
- Full name and address
- Social Security Number or Tnumber [Find your Tnumber]
- Address or fax number where you need the verification sent
