Teacher Credential File Information

School districts will rarely consider a teacher until they have the opportunity to thoroughly review the credential file. Therefore it is in your interest to prepare your file in a way that will demonstrate your knowledge, competence, and professionalism. Failure to do so will create obstacles for you which may be impossible to overcome. Spelling, typing and grammatical errors, make very poor impressions. Carefully proofread what you have written.

In the past SUU has managed the student's credential file.  As of August 1, 2013, all students are required to self-manage their own files. Credential Files set up before this date, are still on file with SUU and can be accessed by phoning our office at 435-586-5420 or emailing Dale Orton orton@suu.edu or Vicki Nielsen vickinielsen@suu.edu.

Self-Managed Files allow candidates the flexibility to adapt the application to the method requested by each school district and give candidates complete control over the process. Credential files contain the following information.

  • Self-Managed Cover Page - If you do not have a Professional Version of Adobe Acrobat then you will not be able to fill out the fields digitally. All computers on SUU campuses have Adobe Acrobat Pro. available.
  • Student Statement
  • Personal Data
  • Resume
  • Courses and Grades Sheet
  • Teacher Evaluations
  • 3-5 letters of recommendation
  • Educator License
  • Praxis Scores
  • Academic Transcript
  • Student Preference Form will be used by our office to help school districts that may call asking for students names who may be interested in filling a position that opened last minute. This is the only form we keep on file in our office (ST-205).

Note: The Word and Excel files work best if you download them to your computer before working on them.
